Layer 1 · Measure the cognitive-risk markers
Standard physicals don't run the markers that actually predict cognitive trajectory. The four that matter most, with the published evidence:
- Homocysteine — elevated homocysteine (>10 µmol/L) predicts accelerated brain atrophy in the Oxford VITACOG trial. Treatable with B12 + folate + B6 supplementation in deficient patients.

- ApoE genotype — ApoE4 carriers (~25% of population) face 3-12× higher Alzheimer's risk. Knowing your genotype changes the urgency of every other intervention.
- Fasting insulin + HbA1c — Insulin resistance is the strongest reversible Alzheimer's risk factor ("Type 3 diabetes" hypothesis). Most doctors only run fasting glucose, which misses the metabolic dysfunction for 5-10 years before it becomes diabetes.

Most cognitive supplements have weak data.
