Why VO2 max is the strongest longevity predictor

The Mandsager et al cohort (Cleveland Clinic, JAMA Network Open 2018) followed 122,007 adults. Adults in the lowest VO2 max quintile had 5× higher all-cause mortality than the highest quintile — stronger than smoking status, hypertension, diabetes, or end-stage renal disease.

Muscle is the second pillar. Janssen et al on sarcopenia: significant muscle loss = 2-3× higher mortality and dramatically higher fall/fracture/dependency risk. Muscle isn't aesthetics — it's a metabolic organ.
